Text Box: Anyone can make a referral to our agency! Our phone lines are staffed 24 hours a day for your convenience.      Intake Hotline: (941) 955-4600 or Toll Free 1(866) 681-0605      Once a referral has been made, our Intake RN will gather all of the information we need to start your treatment. She will also obtain any necessary physician orders to begin care. Our professional Home Care Coordinator will meet with you and your family to assist with your discharge arrangements and with your transfer back home. Our staff will remain in communication with you from the moment your visits have been authorized! Your Personal Scheduler will call to introduce himself/herself and let you know which of our nurses, aides, or therapists, have been assigned to your case. Each of your caregivers will also call you to introduce himself/herself and schedule a time to provide your services!
